Member Forms

The form is a request until management signs the form. There is a $19 cancellation processing fee.

The account must be current and out of term in order to cancel with a 31-day written notice.

All Cancellations require a 31-day written notice at The Ohio Gym, before the first of each month.

The processing fee and any draft with in the 31-day notice must be collected at the time of cancellation.

All freezes must be received 10 days before next draft date.

Maximum freeze is 3 months without a doctor’s note.

Memberships can only be frozen 2 times per calendar year.

The account must have a zero balance. Even if the account is on a freeze the bi-annual fee will still be charged.

This is a member information change form.

This is used for any changes to the members demographic information or billing information.
